Analisis Risiko Internal Leakage pada LPG Treating Unit Menggunakan Metode Failure Mode and Effects Analysis

Internal leakage in LPG Treating Units could reduce process stability, increase product loss, and weaken operational safety barriers. This study aimed to analyze the risk of internal leakage in valve systems and pressure containment equipment using Failure Mode and Effects Analysis. Data were obtained through field observations, semi-structured interviews, historical failure records, and operating parameters. Five failure modes were identified, including LPG control valve passing, internal leakage in emergency shutdown valves, nitrogen valve passing, pressure safety valve seat leakage, and flange connection leakage. The results showed that LPG control valve passing had the highest risk value of 336, followed by emergency shutdown valve internal leakage with a value of 315. Both failure modes were classified as high-risk conditions because they affected pressure stability, product loss, and emergency isolation reliability. Mitigation through predictive maintenance, periodic inspection, and seat and seal replacement was estimated to reduce the risk level by more than 50%.
